Park Waiver Forms

Guests planning to enjoy WildPlay’s ZOOM Zip Lines or Monkido® Aerial Adventure or the Mt. Sima Bike Park must complete a signed waiver form. To speed up your check-in process, you can download, print and complete the form ahead of time.

Important note: all guests under the age of 19 require a parent or legal guardian to sign their waiver.

Don’t forget to bring your waiver form on your play day!
